Dental and Vision Insurance

Note for members with current Blue Harbor/MetLife plans:

Because of the agreement with AMBA, no new participants will be accepted in the Blue Harbor/MetLife plans.  The MetLife plans are not being discontinued for current participants.  The change forms for MetLife below are still good for current insurance participants only.   If you have this insurance and have questions, call the MRSPA office.

The following are the coverage levels available to you for both dental and vision plans:

MetLife Change Form

If you wish to make any plan changes, change coverage tiers, or add dental or vision, please print and complete the Change Form.  You may fax the forms to 410-512-3840, or mail the forms to BenefitMall, P.O. Box 42827, Baltimore, MD 21284-2827; Attention: Enrollment Department.
